• After submitting a license application, you will be sent a confirmation email from NIPR containing a link to the Fingerprint Authorization Form. Download the authorization form and bring it to an approved fingerprint vendor, then upload the completed form to your application after being fingerprinted.

For any criminal history, attach all required supporting documents including:   

  • Original arrest/charging document,

  • Final disposition document,

  • Personal written statement explaining the incident.
